The Australian Commission on Safety and Quality in Health Care (The Commission) is a government agency that leads and coordinates national improvements in safety and quality in health care across Australia. Our aim is to support healthcare professionals, organisations and policy makers who work with patients and carers. The Commission developed the National Safety and Quality Health Service Standards (2011), and is committed to supporting clinicians in their use. Engaging patients in communication at transitions in care is key to meeting a number of these safety standards such as Standard 2 partnering with consumers, Standard 6 clinical handover and Standard 9 recognising and responding to clinical deterioration.

The Commission has appointed a consortium of researchers from Deakin and Griffith Universities led by Professors Tracey Bucknall and Wendy Chaboyer to undertake a literature review and key stakeholder interviews with clinicians who have involved patients/families in communication regarding patients’ transition from hospital to home. Understanding the strategies clinicians and consumers use to promote conversations at transitions of care will inform the future development of resources to assist clinicians, patients and their families to engage in communication at transitions of care into, within and out of acute health facilities.

A very successful awards night at the recent Gold Coast Health and Medical Research conference. Prof Wendy Chaboyer won the Excellence in Research Leadership award for 2013. Prof Claire Rickard and Dr Li Zhang were awarded HPI Publication of the Year 2013. Congratulations.
