Parents of Blended Learning Students (Groups A, B & C only),

In order to open the schools safely on December 7th for our in-person students and staff members, the NYC DOE is requiring that all students in grades 1 and higher consent to testing by their first scheduled in-person learning day.

Families can submit consent using NYCSA (New York City Schools Account) or this updated paper form.

Students who do not submit consent will be transitioned to remote only learning.

If you have not set up your New York City Schools Account (NYSCA), please contact [email protected] and I will send you the Creation Code. You will also need to set up this account to view your child's report card as well as attendance, biographical information and to receive important messages directly from the NYCDOE.

NYC Public schools will reopen for students in Blended learning (grades K- grade 5) on Monday, December 7th.

Wednesday, December 9th - Group A
Thursday, December 10th - Group B
Friday, December 11th - Group C

Please note all Blended students returning to school for in-person learning must complete the Consent Form for COVID Testing (grades 1-5). This can be done on the NYSCA account https://mystudent.nyc (Go into manage account and on the dropdown choose consent form). If you have not created your NYSCA account, please contact Mrs. Lipton at [email protected] and she will send you the creation code to create your account.

Even if you have submitted the consent form previously, please complete again so it is the most updated.

On Monday, Nov. 23 and Tuesday, Nov. 24 we will be distributing the second set of books to our remote students.

10:15 - 11:30, grade 1

8:45 - 10:00, grade 3
10:15 - 11:30, grade 5
