Paranoid Personality Disorder is characterized by a pervasive distrust and suspiciousness of other people.

People with this disorder assume that others are out to harm them, take advantage of them, or humiliate them in some way. They are constantly suspicious of others when there is no reason, have a tendency towards hostility, and put a lot of effort into protecting themselves and keeping their distance from others.

They are known to preemptively attack others whom they feel threatened by. They tend to hold grudges, are litigious, and display pathological jealously.

Distorted thinking is evident, with eagerness to observe others to validate the distorted delusional thoughts. Their perception of the environment includes reading malevolent intentions into genuinely harmless, innocuous comments or behavior, and dwelling on past slights. They tend to experience excessive self-importance, and manifest a persistent self-referential attitude.

For these reasons, they do not confide in others and do not allow themselves to develop close relationships. Their emotional life tends to be dominated by distrust and hostility, and therefore have constricted emotional lives.

Just had a patient this month with rhabdomyolysis. The classic triad of myalgia (muscle pain), generalized weakness and dark urine should alert one to possible diagnosis of rhabdomyolysis.

It is frequently caused by prescribed medications (such as statins for high cholesterol), alcohol abuse leading to unresponsiveness, illicit drug use, heatstroke, infection, trauma, metabolic disorders, and inflammatory myopathies.

Treatment consist of improving nutrition, fluid rehydration, avoiding toxic substances such as alcohol & drugs (prescribed or illicit), correcting electrolyte imbalances, with prevention of kidney failure, compartment syndrome, and DIC (a late sign).

Sliding-Scale Insulin: Time to Say Goodbye? 04/15/2017

The American Diabetes Association (ADA) does not recommend sliding scale insulin (SSI). It's the leading cause of hypoglycemic episodes. SSI is an outdated practice. Providers and facilities utilizing SSI are encouraged to use evidenced-based practice guidelines with basal-bolus (B-Bl) coverage for patients with diabetes.

Sliding-Scale Insulin: Time to Say Goodbye? A recent retrospective analysis showed that nearly 75% of long-term care patients with diabetes received SSI.
