Featured Case Study!

A 50-year-old male with systemic lupus on mycophenolate mofetil presents with the above picture rash which began 3 days prior. A punch biopsy is performed. Histopathology shows an intraepidermal vesicle with acantholysis, ballooning degeneration of keratinocytes, multinucleated giant cells with steel-gray nuclei, and eosinophilic intranuclear inclusions with peripheral margination of chromatin.

Which of the following statements about these histopathologic findings is correct?

1. The intranuclear inclusions (Cowdry type A bodies) are specific for varicella-zoster virus and exclude herpes simplex virus
2. Immunohistochemistry or PCR is required to definitively distinguish VZV from HSV on biopsy, as the histopathologic findings are otherwise indistinguishable
3. The presence of eosinophils in the biopsy specimen would argue against a herpetic etiology and suggest an arthropod bite reaction
4. A subepidermal vesicle with a linear band of IgG at the basement membrane zone on direct immunofluorescence would be expected
5. Tzanck smear is more sensitive than PCR for confirming VZV infection
