Having a swallowing or mealtime plan on file can create a sense of security.

But here's the uncomfortable question:

How confident are you that every staff member knows they're working from the correct, current recommendations?

In busy aged care and disability environments, teams change. Information is handed over. Documents are updated.

And when staff don't have a strong understanding of dysphagia and IDDSI, would they even recognise when something doesn't seem right?

For one man Barbara supported, that simple request represented far more than food.

He was living with a degenerative neurological condition and had swallowing recommendations that significantly restricted what he could safely eat and drink.

But he missed going to the cafรฉ and eating the foods he enjoyed.

Can you blame him?

The difficulty was that previous attempts to eat those foods had resulted in serious choking incidents.

Suddenly, the people supporting him were faced with a complicated question:

What happens when choice and swallowing safety collide?

โ€œShe was tired of being told what she couldnโ€™t eat.โ€

Barbara once met a woman in her early 40s living with a lifelong disability who had experienced coughing and choking incidents during meals.

When Barbara arrived to complete a swallowing assessment, the woman wanted nothing to do with it.

And perhaps that wasn't surprising.

She had spent much of her life being assessed, supported and told what she shouldโ€”or shouldn'tโ€”do.

This time, Barbara didn't begin by pushing through an assessment.

She listened.

They talked about the foods the woman missed, her frustration with modified meals and what eating meant to her.

That conversation changed everything.

Trust developed. The assessment went ahead, and they were able to find a way forward that the woman felt comfortable with.

It's a response Barbara Braithwaite has heard repeatedly during more than 40 years working with adults in aged care and disability.

Someone coughs during a meal.

A staff member noticesโ€”but because it happens regularly, it has gradually become accepted as simply *what that person does.*

And that's concerning.

When something happens every day, it's easy for it to become part of the background.

But would your staff know when a seemingly familiar behaviour could indicate a potential swallowing problem?

She simply wasn't interested in eating.

A carer working in aged care was struggling, at mealtimes, to engage a resident living with dementia..

She cared. She was trying. But nothing seemed to work.

Then she completed our Safe Swallowing Education course.

One simple suggestion from the Course made her reconsider the resident's mealtime environment.

She made a small changeโ€”and suddenly the resident was able to engage with her meal.

A staff member once said this to Barbara after preparing a regular cup of tea for a resident who had been prescribed thickened fluids.

It wasn't because she didn't care.

She simply didn't understand.

And that's what makes gaps in dysphagia knowledge so concerning.

You can have care plans, swallowing recommendations and procedures in placeโ€”but do the people providing everyday care truly understand them?
