ITS TIME TO START HOLDING FEET TO THE FIRE - SPORE TESTING VALIDATION FOR YOUR STERILISER

At the moment you are being told that you are not to use remote spore testing, period! We are asking: "why not"? We can never get a definitive answer from any company, organisation or association.

Think about when you are being told things in your dental infection control. What evidence are you being provided about spore testing? Nothing. Zero.

There are two messages here:

1. Associations and 'advisors': do what we say, when we say, because our unqualified, non-scientific committees have come up with nonsense and put it into a document

2. Companies and services: We love to make huge profits from your dental practice, and if you find a cheaper alternative then we will not like this and will push back because how dare you find a way to reduce your cost at our expense

There are 4 simple questions to ask about these spore tests. Try and see if anyone can answer them for you definitively.

In the end the advice you are given is completely false and unvalidated. Lets hold their feet to the fire and see if they can answer these very very simple questions. If not, then at least you know where you will stand:

1. Which exact section inside AS 5369:2023 mandates technicians (please highlight the word “technician”), to perform annual spore testing validation?

2. What is the name of the ISO referencing for spore testing for small b-class sterilisers and what type of spore is required over what incubation time

3. Please indicate which section of any legislation that “mandates” the use of AS 5369:2023 for office based dental practices

4. If a practices adheres to your advice, can you please provide a disclaimer of liability indicating that the ADA will be liable for this advice given if any infection were to occur

Instead of one company rebutting to another, lets just ask the question and see if answers can be provided.

PATIENTS ARE AT RISK AND NO ONE IS TALKING ABOUT IT

You are told to put a plastic helix device into your steriliser and then place a class 6 indicator into your cycle load.

By doing this you are NOT validating your handpieces.

Remember that air bubbles will block steam from sterilising your handpieces, so you will need a device and indicator that can monitor this. Plastic helixes run once a day and a class 6 indicator cannot detect these parameters.

For those who are telling you they can, ensure you request this in writing from them and also request them to take liability for what they are claiming.

It sounds strange, but the science on this has been around since 1966, and we have moved away from this technology in dentistry.

Companies are doing nothing about but following suit. They will see a plastic helix device and just copy it and sell it for less.

Then information will come out in the market about perhaps a metallic helix will be better than a plastic helix, so they engage companies overseas to make a cheap version of this helix, hence they will sell you an aluminium cheap $5 device for $200-$300.
But none of them understand what these helix devices do, and none of these devices can validate any of your handpieces.

Ensure that your helix devices are properly validated, properly aligned with the right materials and used in each cycle with a proper Class II indicator that has the capability to detect critical parameters inside your handpieces.

MOST INFECTION CONTROL IS UNECESSARY FOR DENTAL

There are a flurry of courses, online seminars and information out there which sounds great, but is it necessary? Probably not. Can it be validated? Probably not.

Even guidelines are incredibly misleading with a lot of information in them that are just not validated for your practice, and much of it is completely unnecessary.

Yes, we know there will be a lot of push back from companies, ADA, techs etc. The fact is, they will never contact us, or the science team, or ISO, or standards Australia to validate. They just contact dentists and try and convince them otherwise.

But the facts are that your practice does not require much of what is being pushed, and a lot of this stuff comes at a huge cost to your business.

Think of two things:

1. Compliance - there is not much compliance required for your infection control because you are not a hospital with theatres requiring changerooms and isolations units

2. Cash. You are spending so much money on things you do not need. If your current consumable cost is $100,000 a year then you are spending $70,000 more than you have to

You will never be taught these things, because from a product sales perspective its bad business. But dentists should be aware of these things as you are simply paying for things you do not need.
Its not that sales companies are pulling the wool over peoples heads, most sales companies are very sincere and want to help their dental customers – no question, but they are simply unaware of these factors.

Here is the truth behind the compliance process

Let's take spores for example.

Are the mandatory? NO

Do they do anything in relation to validation? NO

Are they expensive? YES!

You will be told every story under the sun about how everything is mandatory and how everything must be done in accordance with AS 5369:2023.

There is no point in companies bickering back and forth with each other on this topic, so the best thing for your to do is to ask those who are telling you to comply with all of these guidelines and standards to:

"Put in writing that if you follow their recommendations, and something was to go wrong, or it is discovered that you, in fact, never required these products in order to perform compliance, then these companies or individuals will take FULL LIABILITY for any infection spread that may occur, or any liability towards you or your practice"

You are spending so much money on useless things you simply do no need, that are also massively overpriced, and most of these products do nothing for you or your practice.

Remember that these products are very PROFITABLE for companies and they only go on the attack when they are losing cash. Why should you spend money out of your pocket when you don't have to?

So its best to have everything in writing.

Remember the National Law tells you to get all of your methods from AS 5369:2023, but then AS 5369:2023 tells you to reference the ISO standards.

Everything else is just hearsay.
