Lignocaine is one of the most used local anesthetic drug in health facilities,it comes in a presentation of 2%,1%.Knowing it's presentation is very important,there is one with Adrenaline and another without

Dosages
Plain=3mg/kgbwt
With Adrenaline=7mg/kgbwt
These are maximum doses,do not exceed

At OPD,use a plain one

2% means 2000mg of Lidocaine was dissolved in 100mls of water for injection and;
1% means 1000mg of Lidocaine was dissolved in 100mls of water for injection

E.g.2% lignocaine
60kg woman who sustains an injury(plain)
60*3=180mg
Therefore;

2000mg ➡️ 100mls
180mg ➡️(180mg*100mls)/2000mg
=9mls
Meaning: if you are using 2%,you can not give more than 9mls=best avoid 2%

E.g.1% lignocaine
60kg woman who sustains an injury(plain)
60*3=180mg
Therefore;

1000mg ➡️ 100mls
180mg ➡️(180mg*100mls)/1000mg
=18mls
Meaning: if using 1%,you can not give more than 18mls

E.g.0.5% lignocaine
60kg woman who sustains an injury(plain)
60*3=180mg
Therefore;

500mg ➡️ 100mls
180mg ➡️(180mg*100mls)/500mg
=36mls
Meaning: if using 0.5%,you cannot give more than 36mls.

Avoid Lignocaine in the following
1. P***s P***s
2. 👃 Tip of the nose
3. 👂 Lobes
4. 🤌 Fingers

In short ;
To make 1% from 2% Lignocaine
➡️ Add 1ml of sterile water for injection to 1mls 2% Lignocaine.

To make 0.5% from 2% Lignocaine
➡️ Add 2ml of sterile water for injection to 1mls 2% Lignocaine

*Fluid Maintenance*
Fluid maintenance is cardinal for everyone,and very sick pts also require this.So to calculate Fluid Maintenance,the following are the calculation.
But before we calculate the maintenance,wt is very important,otherwise risk overloading or give less.
So to calculate the maintenance,the following formulas apply based on the weights.
1. BWT upto 10kg= 100mls/kgbwt/24 hrs,e.g a child weighing 9 kg, Maintenance = 9*100= 900mls/24hrs of which you can round off to *1000mls*

2.BWT(10-20kg),Formula 2= 100mls/kgbwt + 50mls * every wt after 10kgs,e.g a 5 yrs old Child=(5+4)2= 18kg, therefore it will be 100mls *10= 1000mls + 5*8= 400mls,so the total for this child in 24hrs will be = *1400mls(1500mls)*

3. BWT > 20kg, Formular is F1+F2+20mls* by every weight after 20 kgs,example a 10 yrs old who needs maintenance,calculate and then do the same adult ♂️ and adult ♀️.
