1.Indemnity form (get at crèche)
2.Enrollment form (get at crèche
3. Affidavit form (get at crèche)
4. 2* Copies of childs birth certificate
5.2* Copies of childs immunisation card from health care.

NB!!!
ALL FORMS TO BE HANDED IN AS REQUIRED FOR CHILD TO BE ADMITTED. OUTSTANDING/MISSING FORMS WILL NOT GRANT YOUR CHILD A PLACE HERE AT THE SCHOOL...
