Absolute deadline for receiving the completed questionnaire is September 8, 2021. All information provided on the questionnaire will be printed in the booklet. Mail your questionnaire to Ilene W. Devlin, 27 Inwood Manor, San Antonio, TX 78248-1631. You can also e-mail a pdf to [email protected].

All who return a questionnaire will be mailed a free booklet. All questionnaires received after September 8 will be added as loose sheets to the bound booklet.
