Nothing in medicine is black in white, so there will always be exceptions to this. But in general, waiting for the ER to provide care that the patient needs but is readily available to us is not a prime example of patient care and advocacy. If you have the tools and the time, use them.

Some time ago, we had a patient die in our care that really got to me. It felt unfair and unnecessary. We did everything we could, but it wasn’t enough.

We had the patient’s last viable EKG in our records. I reached out to our program compliance department and asked for permission to create these vials that are sometimes provided to families in-hospital, and can be exceptionally meaningful to some loved ones. With permission and approval from both my program and the patient’s family (an important piece, as not everyone will benefit from this gesture), I stuck these vials in the mail.

This wasn’t an entirely altruistic move. I needed - for me - for the family to know that the people who were with their loved one in their last moments really cared. We did everything we could. We tried, and we’re so sorry it wasn’t enough. They didn’t die alone. I hope the message was received.

Anywho, this is a reminder that empathy for our patients shouldn’t dissipate with time on the job. Seeing the humans we care for as real people who are an integral part of their loved ones’ lives is crucial to providing compassionate care. Take advantage of the perspective it provides; we’re lucky to be constantly reminded of all we have to lose.
