🏈 Attention Parents of Student Athletes 🏐

Before your child can participate in any school sport, all required Rank One forms must be completed. These forms include important medical information, emergency contacts, UIL-required paperwork, and parent acknowledgments. Students cannot practice or compete until all required forms have been submitted and approved.

Please complete your child's Rank One forms as soon as possible to avoid any delays in participation. If you need assistance, the directions are below. Thank you for helping us keep our student-athletes safe and ready for the season!

Instructions to complete WFISD Athletic Forms online

The web address is: https://wichitafallsisd.rankone.com/New/NewFormList.aspx

Click on the form name and fill out the information requested. You must have your student's ID number available as it is required on several forms.

To sign the document, click inside the signature box and hold your left mouse button down. This will allow you to create an “Electronic Signature.” If a mistake is made and you need to start over, click on the refresh icon next to the signature box.

Once you have filled out all the required information, click the submit button. Make sure you click the submit button when finished with each page.

PLEASE ENSURE YOU SELECT THE CORRECT SCHOOL FOR THE 2026-2027 SCHOOL YEAR FOR YOUR ATHLETE.

DO NOT USE ATHLETE NICKNAMES ON FORMS. You must have their exact name registered under the school ID.

If you have any questions, please contact your respective Head Coach or Band Director.

Please note (as of 07/01/2026) that there is a PAPF question at the beginning of the forms page. Parents must answer that question and then proceed to the forms.

For incoming 7th graders who need immunizations, Community Health Care will be on-site. Remember, students must be compliant with immunizations per the state before being enrolled. Middle school campus nurses, please work out a process with your campus admin to ensure that noncompliant students are cleared by the nurse before being given their schedule.

Last FREE Sports Physical Clinic at Community Healthcare Center (CHC) for 2026!
Date: Thursday, August 6th
Time: 2:00 PM to 5:00 PM
Location: 200 MLK Jr. Blvd., Wichita Falls, TX 76301

This FREE Sports Physical Clinic is open to all middle school and high school students, including those participating in youth sports leagues.

Important Information:
Open to students from all public school districts and private schools, including those who participate in youth sports leagues.
Bring your completed sports physical form signed by a parent or guardian.
Students who wear glasses or contact lenses must have them on hand to pass the vision screening.
Students do not need to be established patients of CHC to receive a sports physical at this event.
