RXVIP Cares is an Education and Career Development Program for PharmDs, Pharmacists and PharmD students that will enhance their role as providers of Patient-Centric Health Care Solutions. RXVIP Cares is a new learning model that integrates education with real world experiential clinical pharmacy experiences. The focus of RXVIP Cares will be to develop skill sets in Care Coordination & Chronic Care

Management to redefine the way Pharmacists can practice professionally, to help deliver better patient outcomes. The RXVIP Cares program will benefit PharmD Students, Pharmacy Managers, Staff Pharmacists, Alumni as well as Physicians and Accountable Care Organizations (ACOs) across Long Island who participate in these clinical practice site environments.
