Send the SAMPLE OF STUDENT PORTFOLIO to me via Facebook Messenger or
e-mail [email protected] including:
1) HOMESCHOOL INFORMATION- Parent name, e-mail address and phone number, county registered with, student name and DOB, the home address you are registered with, and teacher's name (if different than parent). Please include a brief paragraph description of your homeschool.
2) BOOKS- A minimum of 2 book titles/ reading materials used for each student; cookbooks, novels, car manual, textbooks, reference books, magazines, boxed or online curriculum, websites, etc. This does not have to be a complete book list, just a sampling.
3) SAMPLES- A minimum of 6-12 Photos/ samples showing improvement in one subject (minimally) per student. Tests, art, writing, photos from field trips, vacations, pre/ post test, rough/ final draft of writing, math page from the beginning/ end of year, karate belt/ dance class promotion picture, voice lessons, music learned, recipe attempted, artwork completed, sport achievement, character achievement, YouTube channel created, business achievement, agricultural activities, etc. You may also choose to send me a video clip of you flipping through the portfolio, OR you may choose the video call option and simply show me samples during our call while I take notes. After I have received all of the above and we have had our call, I immediately e-mail you your official paperwork. You forward that attachment to your county's Home Education Department. Done--- You are all set for another year!!!
