Iliac vein compression can be caused by intrinsic or extrinsic factors. This compression can lead to serious complications, especially when it's extrinsic, and not part of normal anatomy. Examples include cysts, fibroids, and orthopedic hardware.

The first image is a good reference for extrinsic compression of the left common iliac vein by the left common iliac artery aneurysm via b-mode and color flow Doppler. The second image is a good reference for significantly increased venous flow velocities via spectral pulsed wave Doppler display through the area of compression.

Imaging the abdominal and pelvic veins can be challenging, even for experienced sonographers. To improve your imaging and diagnostic capabilities, consider using additional techniques such as various types of transducers and imaging the patient's flank while directing the ultrasound beam perpendicular to the vessel. These techniques can be valuable when image optimization tools and supine positioning aren't enough.
