The Society of Dermatology Physician Assistants is making an 11th hour push to oppose these changes, which pose a severe threat to derm amongst other specialties and our patients. I am writing to urgently plead for support, as the deadline for comments is quickly approaching on 09/14/26.
The 2 major issues:
The Same-Day Reduction Rule: This rule will decrease your office visit reimbursement by 50% if you perform any same-day procedure, including routine biopsies and cryodestruction.
Overall Derm Reimbursement Cuts: Overall reimbursement cuts across all of dermatology will go from 9% to 11%. This places dermatology in the top two hardest-hit disciplines of all medical specialties.
The implications of these cuts are devastating. Institutions may begin mandating that you bring patients back on a separate day for a biopsy just to avoid the reduction rule. This will invariably disrupt the continuity of care, compromise patient outcomes, and destroy clinical efficiency. Conversely, if you choose to absorb the hit and perform the same-day biopsy, the financial strain will eventually become completely unsustainable for many practices.
