Martha Lloyd Community Services LPN Job Description
The overall responsibilities of the Licensed Practical Nurse (LPN) are to provide nursing interventions to
ensure that the health needs of the individuals are met. All tasks and interventions are focused on
ensuring the optimal health status of individuals, and completed according to established policies and
procedures. The LPN is responsible for the daily health, maintenance and prevention of disease and injury
of the individuals by carrying out the physician’s orders and in accordance with the ICF/ID regulations.
The position of the LPN requires availability to work various shifts, weekends, holidays and on-call.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Nursing
• Assures that the general health status of all individuals is maintained, providing care and
treatment in the event of illness or injury in order to bring the person back to optimal health
status. Provides nursing assessments to individuals as scheduled, and in response to a health
concern or injury. Reports findings to Health Services Supervisor, physician or others as
needed.
• Obtains and records a variety of individual health information including weights, me**es, and
vital signs according to established protocols. Notifies members of the Interdisciplinary Team
of findings as needed (supervisor, physician, dietician, QIDP and other administrative
personnel.)
• Responds in a timely manner to any health concerns reported by individuals or staff members,
providing feedback as appropriate.
• Ensures that each individual has an annual physical examination, dental examination,
specialty examinations and diagnostic services as required by applicable regulations,
physician orders and the individual’s needs.
• Accompanies individuals to appointments, and endures meal/medications arrangements
have been made.
• Maintains sufficient supplies including medications; orders replacements or notifies
supervisor of needs.
• Provides supervision and support to individuals who are ill and confined to the home, making
sure adequate liquids and/or food is provided assisting with toileting, taking vital signs at
appropriate intervals and providing symptom relief.
• Responsible for medication administration in accordance with applicable state regulations
and company policy. Follows all established standards and protocols for counting medications

including narcotics, notifying the Health Services Supervisor immediately of any
discrepancies.
• Takes appropriate action and follows physician orders in the event of a medication error.
Documentation
• Schedules appointments, tests, labs, etc. in a timely manner. Notifies Health Services
Supervisor of difficulties doing so. Charts results of appointments, and follows up with
physician orders.
• Maintains all individual medical records, assuring that they are current and in compliance with
Department of Human Services, Department of Health and other applicable standards.
• Maintains all necessary medication documentation including narcotic logs.
• As needed, obtains physician orders for tests, labs, etc., documents in individual’s charts and
follows through on required actions.
• Ensures that all documentation is completed accurately and in a timely manner.
• Clocks in and out before and after every shift and submits leave request forms when
applicable.
Communication
• Act and communicate in a professional manner.
• Communicates with physician or other health care professional regarding health concerns, or
other questions regarding individuals’ care.
• Notifies appropriate person(s) of upcoming appointments to ensure individuals are ready and
pre-appointment instructions are followed.
• Communicates with individuals, families, staff and administration so that all members of the
MLCS team have adequate information and input to provide the best possible medical care
for the individuals.
• Reports all medication errors to Health Services Supervisor and/or AOC and notifies physician.
• Acts as a representative for MLCS in a professional manner when required, to families, service
coordinators, state officials, community partners, local businesses, and employers.
• Maintains confidentiality of all protected health information and personal information
concerning individuals.
Teamwork
• Attends a variety of meetings for the purpose of providing healthcare information, health
status of individuals, and nursing perspective. Provides training as needed to staff on a variety
of health topics.
• Serves as Nurse On Call according to published schedules. Completes documentation of
occurrences in accordance with established procedures.
• Treats all individuals and coworkers with dignity, respect and fairness.
• Provides support to other team members when individuals are having behavioral issues and
at other times as needed.
Training
• Completes and maintains all required licenses and certifications such as LPN License, First
Aid/CPR Trainer, Medication Administration, etc..
• Responsible for completing 24 hours of in-service training annually, including all mandatory
trainings.

• Attends meetings and training sessions as required.
• Completes annual trainings as required by regulatory agencies.
Safety
• Responds to emergencies in accordance with company policies, procedures and training.
• Reports unsafe conditions, environment, and equipment to Supervisor or AOC.
• Follows safety protocols to foster a safe working environment.
• Keeps work area neat and organized; maintains work area in a sanitary status at all times.
May, on occasion, have to use personal vehicle for work-related purposes.
Performs all other duties as assigned.
JOB REQUIREMENTS
• Be 18 years of age.
• Be a graduate of an Accredited School of Nursing and possess a valid, current license as a
Licensed Practical Nurse.
• Experience in nursing and working with individuals with intellectual disabilities preferred;
• Intermediate computer skills.
• Able to perform essential duties listed above.
• Able to work on weekends, holidays and during inclement weather when required.
• Pass post-offer physical exam: Safety lift/push/pull/carry 50+ lbs. Constant bending, sitting,
standing, walking, lifting, carrying, twisting and reaching. Frequent stooping, squatting,
pushing, pulling and upper extremity grasping. Occasional climbing, kneeling, and driving.
• Pass post-offer drug screen.
• Possess valid driver’s license and have a clean driving record.
