🚨 URGENT: ATTENTION 7TH GRADE PARENTS/GUARDIANS! 🚨🚨
The deadline for required 7th-grade immunization records is Tuesday, September 8!
⚠️ Students who do not have the required records on file will not be able to attend school until the records are submitted.
Per the Ohio Department of Health, incoming 7th-grade students are required to have:
💉 Tdap Booster
💉 Meningococcal (Meningitis) Vaccine
📄 Please submit updated shot records by Tuesday, September 8.

Records may be:
📍 Dropped off at the LHS Office between 7:15 AM–2:30 PM
📠 Faxed to (419) 720-4923
📧 Emailed to [email protected] — Attn: Mrs. Krompak

If your student still needs the required vaccinations, please contact your child’s physician or the Lucas County Health Department at (419) 213-4121 as soon as possible.

🚨 Please do not wait until the deadline. This requirement must be completed for your student to continue attending school.

LHS students in grades 1st – 8th will continue taking the Math iReady Diagnostic Assessment through September 4th. 📊✏️

To help your child succeed, please make sure your Patriot(s):
👉 Is well rested
👉 Eats a healthy breakfast. Breakfast is provided every morning from 7:30 AM to 7:50 AM
👉 Arrives at school on time - 8:00 AM

🚨 Urgent: Attention 7th Grade Parents/Guardians!
The 2026-2027 school year starts this week, please submit your child’s required immunization records for 7th grade.

Per the Ohio Department of Health, all incoming 7th-grade students must have received the required vaccinations:
• Tdap Booster (tetanus, diphtheria, and pertussis)
• Meningitis Vaccine (meningococcal) protects against meningitis

You may send a copy of the records in one of the following ways or bring the updated shot records to school office between 8 AM to 2:30 PM.

• Email: krompak@|hstg.com (Attn: Mrs. Krompak) or drop off the immunization records to the office.

If you have any questions or need assistance, we encourage you to contact your child’s physician or the Lucas County Health Department at (419) 213-4121.
