Hello 5th Grade Families!
The PTA is super excited to sponsor the end of the year party for our Lakewood graduates! The 5th grade party will be held at Ralph Wilson Youth Club on Monday, May 22. Students will be bussed to and from the facility where they will enjoy a day of fun and games. Pizza and cupcakes will be provided for lunch.
***We ask that you complete the following by Friday, April 7.***
1. Every student will receive a free t-shirt that will be given to your child the day of the party. Please complete the attached google form for us to know your child's name and tshirt size. Please double check the spelling of your child's name, as each name will be printed on the t-shirt. This form also includes information about lunch so be sure to let us know if any modifications need to be made for your child.

The PTA provides a special graduation shirt to each fifth grader, free of charge. We are using this form to collect T-shirt sizes. Again, these shirts are at no cost to you, but we need your help for sizing and spelling of names.
2. We are creating a slideshow to play at the party. Please submit ONE photo of your child when they were a baby/younger and ONE current photo to the link below. If you have any technical issues with the link, you can email your photos directly to Amy Thomas at [email protected]. If you send your photos via email, please include your student's first and last name in your email. Also, if you do not have access to a computer or are unable to scan in photos, please let your child's teacher know and we will arrange to get them scanned for you.

3. Only approved volunteers can participate the day of the party. Please submit the BISD volunteer form if you are interested in helping at the party. We will be sending a signup genius form for volunteers soon!

4. In order to make our event even more special, we are collecting donations to pay for extra treats for the kids and teachers and a special lunch for the teachers. You can donate by sending cash with your student in an envelope labeled "5th Grade Party" or you can send via Venmo to -Bossen.
