PHYSICAL THERAPIST (PT)

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced PT for our Facility.

Duties Include:
The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Physical Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Physical Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate physical therapy method.

Requirements/Qualifications:
-Must have graduated from an accredited Physical Therapy college or university.
-Must have a valid professional license and registration for state of practice.
-The Physical Therapist is responsible for overseeing and supervising Physical Therapy Assistants.
-Experience with long-term care and/or geriatric care is preferred but not limited to.
-Well versed with documentation writing
-Evaluations, weekly progress notes, recertification and discharge documentations.
-Collaborates with all disciplines to provide best patient centered treatment and care.
-Exceptional patient service skills and ability to demonstrate clinical excellence
-Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
-Perform all other duties assigned by Director of Rehabilitation
-Compliance with confidentiality, HIPPA and Federal Residents Rights.
-Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

This week several of our 2nd and 3rd year students volunteered their time at the 2026 EmpowerSCI Teen and Young Adult Program took place in East Greenwich, Rhode Island. This one-week residential summer program serves participants ages 14 to 22 with spinal cord injuries, focusing on independence, clinical support, and adaptive recreation (surfing, swimming, cycling, shopping etc).

SB Volunteers included: Jess Lazis, Kayla Campana, Kat DeMirjyn, Alyssa D'Ambrogio, Gabby Oliveri, Makaela West. Also in attendance were SB alumni: Stephanie Romano and Lori Hochman.

This is a TEMPORARY acute care position, Monday thru Friday 8-4 with weekend and holiday rotation responsibilities. Great learning opportunity. Candidate will have mentoring from a Senior Therapist and Lead Therapist. Potential for full time to hire!

DUTIES

1. Plans and provides inpatient physical therapy evaluation and/or treatment programs for a wide variety of diagnoses(joint replacement, trauma, CVA,CABG, LE weakness etc,). Educates to prevent or mitigate disability, injury, disease progression. Treatment includes physical mobilization, therapeutic exercise with or without assistive devices, and the performance and interpretation of tests and measurements to assess physiological, pathomechanical, and developmental deficits of human systems to determine safe treatment, prognosis, continuum of care, and discharge disposition.

2. Coordinates physical therapy programs with other health care personnel ex. occupational therapy, nursing, physicians etc.

3. Directs and aids patients in activities of daily living training, gait training, posture training, therapeutic exercise, joint protection, desensitization, edema control, transitions of care, and various discharge dispositions.

4. Directs and trains patients in the care and use of assistive devices, prosthetic
and orthotic devices including wheelchairs, canes and crutches. Gives instruction in postural control and therapeutic procedures such as neurodevelopment techniques to be continued by the patient and/or parents. Adapts conventional physiotherapeutic techniques to meet the needs of patients unable to comprehend.

5. Evaluates, records and reports patient’s progress for other members of the healthcare/rehabilitation team.

6. May participate in training and evaluation of students in Physical Therapy from the Physical Therapy Program of the Stony Brook’s School of Allied Health Professions as well as physical therapy students and physical therapist assistant students of other affiliated programs.

7. MISC: Participates in weekend and holiday patient care on a rotational basis, consults with physicians concerning goals and treatment plans, participates in departmental in-services and participates in departmental rounds.
