Calling all providers, groups, treatment centers, and clinicians!

Join us on March 27th as clinicians come together for an important conversation about the gaps we’re seeing in coordination of care across providers and levels of treatment.

From day school to outpatient to IOP/PHP, residential, inpatient, and back again, transitions are often where clients/students may be underserved. We’ll be discussing breakdowns in communication, delayed information sharing, and more to shed light on the impact these gaps have on continuity of care and client/student outcome.

This will be a collaborative space to identify patterns, share experiences, and begin building more streamlined, accountable systems of coordination that better support the students and families we serve.

We hope you’ll join us in strengthening our collective approach and we look forward to seeing you there! 😊
