All General Forms must be completed for NEW and RETURNING students in PreK, Kindergarten, 1st Grade, and 2nd Grade.
To complete your forms:

➡️ Click Student & Parent Sign-In
➡️ Select FORMS from the left-hand menu
➡️ Choose General Forms and complete each form listed
📌 Please remember: Updated health records and any required documentation should also be submitted.
🏠 Moved recently? If your address has changed within Somersworth or outside of the city, please contact the school office at 603-692-2435 or email [email protected] with a copy of your new lease/mortgage statement.
