"A new National Academy of Medicine Perspectives paper highlights how this misalignment, driven by fragmented systems, lack of trust, and misaligned incentives, limits progress on health outcomes and equity. The paper outlines key drivers of this misalignment, including limited definitions of health, lack of trust, and poorly aligned financial incentives, and presents actionable solutions to move toward a more people-centered system.

📯 National Quality Forum (NQF) and Joint Commission have just published their updated (2011 to 2025) Serious Reported Events lists. SREs affect all of us one way or another- awareness is a frst step and then how to report?
SREs are very important to be aware of - all healthcare interest holders. Patients to Front Line Staff to Clinicians to Healthcare Facilities and Systems all = Safety Events impact us, our lives. I was honored to have served on one of the TEPS during the development and want to congratulate all the leadership teams for their expertise, guidance and efforts. "Oakbrook Terrace, Illinois, and Washington, DC, January 26, 2026 — Joint Commission and the National Quality Forum (NQF) today announced they are aligning Joint Commission’s Sentinel Events (SE) List and NQF’s recently updated Serious Reportable Events (SRE) List, streamlining safety event reporting and eliminating the need to maintain separate safety measurement frameworks. The intent is to reduce confusion and burden, while placing greater focus on improving patient safety.
Both lists serve as important tools for public reporting of patient safety events. Joint Commission will adopt the updated SRE List as part of the SE List effective January 1, 2027 across all accredited organizations domestic and international, allowing time for healthcare organizations to make the required changes, and will provide technical support to accredited organizations during the transition."
"In addition, to ensure workplace safety throughout Joint Commission’s accredited healthcare organizations, three workforce safety events from the legacy SE List—Homicide, Sexual Abuse/Assault, and Physical Assault of a staff member—will be retained. Together, the patient-focused SRE List along with the three retained workforce safety events will form the SE List beginning in 2027. Joint Commission and NQF will host a patient safety capstone webinar that will cover updates to the SREs on March 3, 2026, 1:00 – 2:15 pm ET."
