District 54 Parents As Teachers 0 to 3 Parent Support

A FREE in-home & community-based parent support program that provides services to families w/kids 0-5

+ Personal Family Support Home Visits
Qualifying families with children 0-3 receive at least 2 home visits per month to support the families in the areas of parent-child engagement, child development, and family well-being.

+ Group Connections & Centers
All families in District 54 with children ages 0-5 are invited to join group activities and playgroups at various locations in the comm

unity.

+ Access to Community Resources
We strive to connect families to the support that they need including Food Pantries, Clothing Closets, Health, Wellness, Safety, Legal, Recreational, and Other Services.

+ Child Screening
Developmental, Health, and Family-Based screenings are conducted every 6 months to partner and reflect on the development of the child and address family needs.
