1st Shift- Van Driver Wanted

Location: Maplewood, MN
Program Hours: 9:00am-3:00pm

Requirements:
A) Pass criminal background check
B) Must gave a valid driver's license with good driving records
C) Must be fluent in Hmong and English.

Program Roles and responsibilities:
A) Assists with the center’s program activities and transport and/or es**rt program participants to, from and within the program, on AADCS grounds, and on external trips.
B) Other roles and responsibilities as assigned.

Hiring Program Coordinator
Hours: 11:30am-8:00pm; Saturday schedule may be in future setting.

Requirements:
A) Preferred AA or Bachelor’s degree from an accredited college/university in Therapeutic Recreation or related field and/or
B) A minimum of 2 years of experience in working with adults with disabilities/functional impairments and/or program planning.
C) Excellent communication skills (written and oral); strong organizational skills.
D) Proficiency in Microsoft Excel, Work PowerPoint and Outlook.
E) The Program Coordinator must demonstrate a positive, professional appearance and attitude, sensitivity to participants’ issues and feelings, and excellent communication skills.
F) Must be fluent in English.

Adult day care Program Coordinator Roles and responsibilities:

A) Responsible for developing and facilitating recreational groups and programs which meet the social, emotional, cognitive, and physical needs of the participants. Lead program activities throughout the days as needed.
B) Assist and serve meals/snacks.
C) Assist w/day to day participant’s needs.
D) Complete enrollment paperwork and read, follow through with member’s mails/correspondences. Contact the appropriate MCO/county/CM to obtain new SA/transfer SA.
Arrange appointments for member and arrange ride and interpreter accordingly.
Monitor SA Request Log (existing members’ SAs). Contact CM or MCOs for missing SAs.
E) Read and respond to daily/regular emails
F) Take Program supply list and order items needed for program use.
G) Track inventories and reconcile records on a weekly basis.
H) Attend weekly team meeting.
I) Attend one-on-one meeting with mgmt. staff
J) Reports to the Program Director and work as an active team member providing support to participants which may include assistance with activities of daily living.
K) The Program Coordinator will help to maintain an organized, sanitary program environment, and ensure that daily supplies for programs and activities are maintained and organized.
L) All essential job responsibilities will be executed in accordance with AADCS policies and compliance/ethics guidelines.
M) Developing groups and programs in consideration of the needs, abilities, and interests of the participants. The provision of programs shall offer a variety of formats including small group, one on one, and large group and take into consideration the multigenerational population in the program.
N) To research new ideas for programs and activities, taking into consideration participants’ suggestions.
O) Initiating and planning the monthly calendar in conjunction with the Program Director/Assistant as well as communicating effectively with the Program Director/Assistant to initiate daily programs and activities.
P) Assists with transporting and/or es**rting program participants to, from and within the program, on AADCS grounds, and on external trips.
Q) All duties must be performed with the highest level of integrity, adhere to AADCS’ Standards of Excellence, and
R) Support principles of person centeredness. All duties will be executed in accordance with AADCS’ policies and procedures and adhere to all regulatory requirements.
S) When needed and requested by member, staff may need to accompany member to appointments.
T) Other related tasks may be assigned as needed.
