📋 Pre-Registration Instructions (For 3 or More Pets):
If you are bringing 3 or more pets, please pre-register several days ahead of time (not the day of) to skip line delays. Email your completed form or pet information to [email protected] so your paperwork can be prepared in advance. On the day of the clinic, bring your pets, get in line, and check in quickly. Information Needed for Pre-Registration: Owner Information: Full Name, Address, and Phone Number. Pet Details (for each pet):Name, Species (Cat or Dog), Breed (for dogs), Hair Length (Short/Long for cats), Color. S*x (Male or Female), Spayed/Neutered status (Neutered, Spayed, or No). Age Bracket (0–3 months, 3–12 months, or Over a year). Weight Bracket (Under 20 lbs, 20–50 lbs, or Over 50 lbs). Selected Vaccines/Services: Indicate required vaccines from the list above.
