Tip Tuesday: Before you prescribe combined oral contraceptives, ask one more question.
Does she get migraines with aura?
Aura — the sensory prodrome before a migraine hits, whether that's a visual disturbance, a smell, a taste, or tingling — is more than a symptom pattern. It's a contraindication to estrogen-containing contraceptives.
Per ACOG and WHO, migraines with aura carry a 16-fold increased risk of stroke. Combined OCPs compound that risk. Progestin-only options exist — but you can't make that call if you don't ask the question.
It's a one-sentence screen that changes the prescription.

Medical joke: what do you call a female on birth control who gets antibiotics for an upper respiratory infection?
Pregnant.
It’s an oldie but a goodie as far as jokes go, but it’s worth understanding why.
Remember that inhibitors slow down metabolism of drugs and inducers speed it up.
Pro tip: be mindful of the patient’s chronic meds when you add temporary meds that may interfere with metabolism, not just the temporary medicines being impacted by the chronic meds.
Chronic meds + temporary meds = always worth a second look.
