🩸 LEFT SHIFT vs. RIGHT SHIFT: What's the difference?

When hemoglobin encounters oxygen, it has two choices: hold on or let go.

⬅️ Left Shift = Hold On
Hemoglobin binds oxygen more tightly, making it harder to deliver oxygen to tissues.

➡️ Right Shift = Let Go
Hemoglobin releases oxygen more easily, increasing oxygen delivery to tissues.

This concept appears throughout EMS education because it helps explain why patients with different conditions can have dramatically different oxygen delivery despite similar oxygen saturation readings.

EMS Scene Time Decisions: Scoop and Run vs. Stay and Play Explained 06/03/2026

🚑 Scoop and Run vs. Stay and Play?

Every EMS provider has heard the debate. The problem is that many clinicians approach it as a rule instead of a decision.

A trauma patient with internal bleeding needs definitive care fast. A patient in severe respiratory distress may need immediate intervention before transport. Knowing the difference is what separates protocol-following from clinical reasoning.

Our latest blog breaks down:
• When scene time should be minimized
• When stabilization takes priority
• How to balance intervention and transport
• Why patient physiology should drive every decision

If you're an EMR, EMT, AEMT, paramedic, or EMS educator, this is a conversation worth revisiting.

Critical incidents do not end when the scene clears.

For EMS providers, the hardest part often starts afterward, when the adrenaline fades, the questions begin, and the emotional weight finally catches up.

This week’s Mental Health Awareness Month blog explores how providers process traumatic calls, why there is no “correct” emotional response, and how health support systems can make a difference after critical incidents.

If you work in EMS, this conversation matters. Read the full blog and share it with someone in your crew who may need it: https://www.impactems.com/ems-processing-critical-trauma/
