Enrollment documentation is due June 30th to secure your scholar's enrollment. The following documents can be submitted by accessing the 2nd Link in our bio (Documentation Upload):
1. Copy of parent/guardian identification
2. Proof of student age (birth certificate, passport, birth record)
3. Copy of Social Security Card
4. Proof of Residency (Deed, Lease Agreement, GA Power Bill less than 30 days old)
5. Immunization Form DPH 32316. Dental, Hearing, Vision DPH Form 3300
