Congress ended surprise medical bills — doctors and insurers hate it

Lawmakers on both sides celebrated in 2020 when, after years of delicate negotiations, Congress passed the No Surprises Act to shield people from the unexpected bills they sometimes received from out-of-network doctors after emergency medical care.

“This must end,” President Donald Trump said when he signed the bill into law. “We’re going to hold insurance companies and hospitals totally accountable.”

Dermatology Practices Absorb Rising Prior Authorization Costs

Dermatology practices are spending a mean of 13 hours a week and filing roughly 39 prior authorization requests per physician annually just to get patients started on medications already prescribed, and still, 1 in 3 requests are denied outright, according to Jane M. No Surprises arbitration payouts hit $15B in 2025 alone: WSJ

The No Surprises Act’s arbitration process awarded close to $15 billion to out-of-network providers in 2025, more than three times the prior year’s total, according to a Wall Street Journal analysis of federal data.

Payouts through the independent dispute resolution process reached $14.85 billion in 2025, up from $4.08 billion in 2024, the Journal reported. Arbitration firms collected about $1.3 billion in fees last year.
