This study presents a two-pronged gene therapy approach that simultaneously boosts peak Na⁺ current and Ca²⁺ transient amplitude in cardiomyocytes to combat heart failure. It works by expressing prokaryotic sodium channels (BacNav), which modulate Na⁺/Ca²⁺ exchanger activity and increase sarcoplasmic reticulum calcium stores. In a mouse pressure-overload model of chronic heart failure, AAV9-delivered BacNav therapy rescued contractile deficit, prevented arrhythmias, and improved transcriptomic dysregulation. Our Cal-520 calcium indicator, was used to measure calcium transients in cardiomyocytes—loaded at 10 µM and imaged to track real-time Ca²⁺ dynamics during pacing experiments.
