Medical Coverage Registration Fair, FRIDAY, SEPTEMBER 27th
9:00am to 4:00pm, Parent Hub, 351 School Street, Pittsburg
North Campus Building, Room 103, across from Pittsburg High School.
Park in lot on Somers Street behind building, in any of our lots, and available street parking.
Enrollment for:
1) Medical
2) Kaiser Community Program
3) Covered CA, and/or
4) Cal-Fresh (support for food)
To apply bring the following documents:
* Identification with photo, name, and date of birth
* Birth Certificate (for minors)
* Proof of income from last month (if you have it)
* Proof of address (if you have it), example: electricity or phone bill, etc.
* Residency card or work permit (if you have it)
* Social Security Card (if you have it)

Edificio del Campus Norte, Sala 103, frente a Pittsburg High School.
Estacionamiento en el lote de Somers Street detrás del edificio, en cualquiera de nuestros lotes y en el estacionamiento disponible en la calle.
Inscripción para:
1) Médical
2) Programa Comunitario Kaiser
3) Covered CA
4) Cal-Fresh (apoyo a la alimentación)
Para aplicar traer los siguientes documentos:
* Identificación con fotografía, nombre y fecha de nacimiento.
* Acta de nacimiento (para menores de edad)
* Comprobante de ingresos del último mes (si lo tiene)
* Comprobante de domicilio (si lo tiene), ejemplo: factura de luz o teléfono, etc.
*Tarjeta de residencia o permiso de trabajo (si lo tiene)
* Tarjeta de Seguro Social (si la tiene)
