We need community involvement for our celebration on August 6th, 2022! The 'Thank You' form is very simple, and it is a way to tell the Foundation who has done so much good in our community, thank you! Our goal is that members from the community who have been impacted by the Clemens Foundation or Rex & Ethel Clemens personally, have a chance to share where the Clemens scholarship has taken them in life. These ‘Thank You’ forms will be given to the Foundation after the celebration. You can find the PDF form at our website. We will also have them printed at the celebration to fill out.
