Are you feeling lost treating people with chronic pain?
Maybe you realize that there is more going on there but feel underprepared to address it?
Maybe you feel like you could be doing more but aren't sure what to do?
In a qualitative study, Synnott et al. demonstrated that:
Physical therapists only partially recognized cognitive, psychological, and social factors. Some physical therapists stigmatized patients with LBP as demanding, attention-seeking, and poorly motivated. And some physical therapists believed screening for BPS factors were beyond scope of practice with many physical therapists feeling under-skilled
Physiotherapists may stigmatize or feel unprepared to treat people with low back pain and psychosocial factors that influence recovery: a systematic review. Synnott A, O'Keeffe M, Bunzli S, Dankaerts W, O'Sullivan P, O'Sullivan K. J Physiother. 2015 Apr;61(2):68-76.

We inadvertently shift our clients' beliefs through education, which is no easy endeavor when met with preconditioned notions. But what about our own beliefs?
Should the goal of our treatment be to improve movement or chase pain reduction (HINT: Which one is more within the realm of our autonomous capability?)?
The struggle for pain control is the single best predictor of: worse pain, poorer activity/greater disability, worse depression, greater avoidance.
Maybe we need to shift our focus from pain to movement, hoping to at least address the peripheral mechanism(s) contributing to a patient's pain. What have we got to lose? If physical therapists can optimize movement with the aims of reducing tissue stresses thought to potentially contribute to nociceptive input, this would be the beginning of one great victory towards pain dechronification.

Yes, there is a lot of science to support the powers of gratitude in combating suffering associated with chronic pain and depression. Yes, these 3 steps may seem simply silly but if you can do them mindfully, we believe you will be able to feel the power if you choose to feel it. Can you do it? Can you see the potential? Turn these into daily habits and notice what happens to your mind.

🤔 We all ask questions about the “what” behind our patient’s background & their goals…

🤔 But, how often do we ask about the “why?”
-
🤔 Why is this important to them? Why is it that they value it?
-
🤔 Maybe our outcomes would be more successful if we truly connected with our patients values & motivations?
-
💯 Research such as this suggests it’s worthwhile to do so!
-
🗣 The next time you’re with a patient, simply try to ask them “why?” until you’ve fully explored all that is driving them in their journey with you!

💡 Did you know that the movie is based on the work of the Dalai Lama & Dr. Paul Ekman & their “Atlas of Emotions?” Not only they, Dr. Ekman was a vital consultant for the film!

🤓 The “Atlas of Emotions” is based on research that has illustrated a consensus among researchers that there are FIVE UNIVERSAL EMOTIONS, which are:
-
1️⃣ FEAR: Our fear of danger lets us anticipate threats to our safety. The message can range from low level concern to conveying panic.
-
2️⃣ ANGER: We get angry when something blocks us or we feel we are being treated unfairly. It can carry a message ranging from dissatisfaction to threat.
-
3️⃣ DISGUST: Feeling disgusted by what is toxic helps us avoid being poisoned, physically or socially. It tends to show that the target of disgust is to be kept away from.
-
4️⃣ SADNESS: Sadness is a response to loss which allows us to take a timeout & show that we need support. It tends to encourage empathy from others.
-
5️⃣ JOY: Joy describes the many pleasant feelings that arise from experiences both familiar & novel. It demonstrates desire to engage in social interaction.
