Comparing Second degree type I AV block and complete heart block on ECG:

Whereas we expect a constant PR interval in first degree AV block, as well as second degree type II AV block:

2️⃣nd degree type I (Mobitz I): PR variable
πŸ‘‰πŸ»Look for a progressive lengthening of the PR interval preceding dropped QRS
πŸ‘‰πŸ»Expect PR to reset following dropped QRS

3️⃣rd degree: PR variable (not a true PR interval due to loss of A to V communication)
πŸ‘‰πŸ»Look for a variable and constantly changing PR interval indicating no communication between atria and ventricles
πŸ‘‰πŸ»Note the QRS may be narrow or wide if the AV junction or ventricles pick up the escape beat, respectively

A-fib versus junctional rhythms. These can be tricky to decipher when first starting ECG interpretation.

1️⃣ Key characteristics of AF: 1) R waves that are irregularly irregular (no way to predict where the next will land) and 2) an absence of discrete/ reliable P waves. The irregularity is attributed to a sporadic conduction of atrial impulses from above.

2️⃣ Junctional rhythms by comparison: 1) May or may not have P waves (if present, expected to be inverted before or after the QRS in lead II) and 2) typically have a regular R wave pattern (marches out) due to origination from a site capable of producing a depolarization stimulus.
