12-lead region evaluation
My preference is to work left to right as if I’m reading a book, simultaneously grouping the leads together in regions that are related.
Using the image above, here’s what that progression looks like:

1. Inferior leads: II, III, aVF

2. Antero-septal regions: V1 – V4 leading into lateral

3. Lateral: V5, V6 into high laterals

4. High lateral: I, aVL (+aVR)
As I’m scanning through each region quickly, I try to make a mental note of any depression or elevation so that I can confirm this with any reciprocal leads that might show mirrored depression or elevation, and this might also change my planned approach (i.e will glaring antero-septal changes immediately draw attention? Probably.). But the approach above is what helped me make sense of the mountain of information the 12 lead provides.

The bundle branch block turn signal:
Using V1 as a starting point, what does the QRS look like? Is it wide and mostly positive or wide and mostly negative?
One of my favorite illustrative concepts to use for this is the V1 steering wheel above. Imagine you’re driving your car (in the U.S., this may differ elsewhere 🚨). You arrive at a four-way stop and need to signal the car directly across from you that you’re making a right-hand turn.
Which way are you going to flip the turn signal: up, or down?
That’s correct, up for right! If you were to make a left-hand turn, the turn signal would be flipped down for left. That’s it!
We’ll need to confirm this information further using additional leads like Lead 1 and V6. The information above is great as a simple memorization trick, but that’s not what we’re here for. We want to understand the underlying concepts for the tricks we’re discussing.
Remember, a positive impulse traveling toward a positive electrode should produce a positive (or biphasic) deflection (V1). A positive impulse traveling away should produce a negative deflection (V1).
