Attention parents and student-athletes:
We are excited to announce starting for the 2020-2021 school year all athletic forms will be digital. Student-athletes will no longer be turning in the paper copy of their physical packets to the athletic training staff. Instead, parents will create an account for their child at https://athleticclearance.fhsaahome.org/. Directions for this process can be found at https://lakenonaathletics.org/ or here using an OCPS account: https://docs.google.com/document/d/16cdC1Wu9_61i9rPoKTDSxCNT2MwETyALmV4_PF8XNcE/edit. In order to complete the profile a picture or scanned image of the EL2 form (sports physical physician forms) must be uploaded. Click here for the EL2 Form. If the student-athlete’s sports physical is still current (it’s good for 365 days) please complete the rest of the profile. Every student-athlete must complete this process in order to get cleared for athletic participation.
