edical Home (PCMH). It is a model of health care "centered" around the patient needs, whether it is a same-day appointment, patient education regarding a particular disease or diagnosis, or a phone call returned in a timely manner. Your child's provider at TCP leads a team of health care professionals who are responsible for your child's health, whether they are all or well. How does PCMH benefit you?
