FINALFORMS REGISTRATION STEPS…
Parents: Please complete ALL of the following steps before your student
participates in a sport.
1. CREATE YOUR FINALFORMS ACCOUNT
Go to your school’s FinalForms website and select New Account / Parent. You can find this on
the Avery High School page under Athletics. (Scroll down the page to the FinalForms tab)
Enter your information and create your account. Check your email and click the confirmation
link to activate your account.
2. ADD YOUR STUDENT
After logging in, select Register Student or Add Student.
Enter your student’s information exactly as it appears in school records.
If you already have a FinalForms account for another child, DO NOT create a new account.
Simply log in and add your new student.
3. SELECT THE SPORT
Select the sport your student will participate in.
If your student will participate in more than one sport, make sure you select each appropriate
sport.
4. COMPLETE ALL REQUIRED INFORMATION & FORMS
Work through the registration forms and complete all required information, including:
• Student information
• Parent/guardian information
• Emergency contacts
• Medical information
• Athletic participation forms
• School-required agreements and policies
Read each form carefully and complete all required parent and student signatures.
5. UPLOAD THE PHYSICAL
If your school requires a physical:
• Have the physical completed by a medical provider.
• Take a clear picture or scan of the completed form.
• Upload it to the Physicals section in FinalForms.
• Make sure ALL pages are included and easy to read.
6. CHECK FOR MISSING ITEMS
Before you finish, go back through your student's FinalForms account.
Make sure there are NO required forms, signatures, or documents left incomplete.
7. WAIT FOR CLEARANCE
Completing FinalForms does NOT automatically mean your student is cleared to
participate.
The athletic department must review the registration and required documents.
Your student should not participate in practices, tryouts, or games until they have been
cleared by the school.
FINAL CHECKLIST
☐ Parent account created and email confirmed
☐ Student added
☐ Sport selected
☐ All required information completed
☐ Parent signatures completed
☐ Student signatures completed
☐ Physical uploaded, if required
☐ All required forms show complete
☐ Student has been cleared by the athletic department
