A QC process doesn't break all at once.

It happens gradually. We see the same five warning signs again and again:

➡️ Errors slip through that a proper review step would have caught

➡️ Senior team members get pulled into day-to-day editing

➡️ Quality drops and nobody catches it early

➡️ Training materials get too heavy or inconsistent for editors to use

➡️ The same errors keep showing up across different contributors

None of these things look like a crisis on their own. Together, they signal a QC process that hasn't kept pace with the content operation.
