The Adult Day Health Care Center at Palm Gardens is a medical model program designed to meet the needs of individuals who are frail, physically limited or isolated. Transportation is made available door to door to pick-up participants and return them safely home. The services provided by the Palm Gardens Adult Day Health Care Center will support ability to remain at home and function with independence and dignity.
