The Montgomery County Health Department will be giving COVID Vaccinations for children age 5-12 years old this Saturday from 8-12.

Flu Vaccination forms will go home today.
Our Flu Vaccine Clinic is October 7th.
The completed forms are due back by October 5th.
Flu mist will more than likely not be available. Please indicate on the form whether you would be okay with your child receiving a shot instead in the case that the mist is not an option.

The Covid vaccine forms for staff and students who have not been vaccinated are due today by the end of school.

The Montgomery County Health Department is coming on 9/3/21 to do a COVID19 vaccination clinic for students 12 and older and Staff (that have not been vaccinated).

Forms went home today.
****Deadline to turn in your form is 8/27/21**********

Some 6th grade students meet the age requirements for the COVID19 vaccine.
These students that meet the age requirements and bring their consent forms back by 8/27/21 will ride a bus to the highschool to get the vaccine.
Those that receive the COVID19 vaccination on 9/3/21 will need a second vaccination.
Paperwork will be sent home at that time (stating the date and the time) of the Second Covid vaccine clinic.
The Pfizer Vaccine is the vaccination that will be given to students and staff on 9/3/21.

Sports physical consent forms went home on Tuesday for 6th-11th grade.
St. Vincent’s clinic is coming to the basketball gym to give these free physicals on May 19th (this coming Wednesday) to our students who bring back their consent form.
Students can turn their consent forms into their building office.
The 6th graders that bring the consent forms back will ride the bus to the highschool gym to get their physical and ride the bus back to the elementary.
This is a free physical provided by St. Vincent’s Clinic to our students.

Sports Physical clinic forms went home on Tuesday.
St Vincent’s clinic of coming to the school May 19th (this coming Wednesday) to do a free sports physical for students who bring back their permission slips signed by their parent.
-students can bring back their permission slips to their building office. At the very latest they can bring their permission slip the day of the clinic.
6th graders who have brought the permission slips signed will ride the bus to the highschool gym and will get their physicals and come back to the elementary.
This is a free Sports Physical for students that St. Vincent’s is doing for our school.
