Jefferson County Health Department-Covid 19 Vaccine Update

We have worked through all of our forms backlog and are ready to schedule vaccine appointments. To schedule a COVID-19 vaccine appointment with the Florida Department of Health in Jefferson, please call (850) 342-0170 and press option 1 to reach the for the COVID-19 hotline. Hotline staff will transfer you to the vaccine schedulers. We are asking everyone to bring the completed consent form to their vaccine appointment at the health department. The form is attached and is available on our website: http://jefferson.flhealth.gov. Copies can be mailed from the health department or one can be obtained from the health department.



We are not using the online ShareCare appointment system at this time.



As a county health department, we are bound by Executive Order 20-315 which is summarized below.



The Executive Order 20-315 which directs us to focus on the following populations during this initial phase of vaccinations:

•Long-term care facility residents and staff;

•Persons 65 years of age and older; and

•Health care personnel with direct patient contact.



The executive order also states that hospital providers may vaccinate persons who they deem to be extremely vulnerable to COVID-19.



As additional vaccine is made available, the vaccines will be provided according to the Governor’s orders to additional groups.
