Chronic subdural hematomas come back after surgery 10–20% of the time. MMA embolization is changing that, and three randomized trials published in the New England Journal of Medicine in November 2024 give us real data.

Think chronic SDH in the older patient who fell weeks ago and now has new confusion, headache, or weakness. Find out whether your neurosurgery team offers MMAE and who they select. MMAE is not a substitute for emergent decompression of an acute SDH with mass effect.

REBOA ends up in the zone you meant about 71% of the time.

We size the catheter with external landmarks and an estimated length, we inflate, and then we hope. In the trauma bay you can get a film to check. On a helicopter, in a rural ED, or downrange, you cannot.

So a team at Brooke Army Medical Center sent the imaging up the artery with the catheter. Ten swine, continuous hemorrhage, REBOA positioned on live intravascular ultrasound. All ten balloons landed correctly, and the operator's experience level did not change that.

Ten animals and no outcome data, so hold it loosely. But the argument underneath is hard to ignore. IVUS is portable. Fluoroscopy is not.

Worth asking at your own center: when the balloon is up, who is confirming the zone, and how?
