Perimenopause is often the “no man’s land” of hormones.

Somewhere along your hormone journey, you’ve probably heard:

“Oral estradiol is bad because it goes through the liver and causes clots.”

But hormone therapy — and the research behind it — is far more nuanced than that.

When oral estradiol passes through the liver, it creates downstream compounds and fatty acid esters that transdermal estrogen largely bypasses.

These compounds may support:
• vascular and endothelial health
• nitric oxide production and blood flow
• HDL and lipid metabolism
• visceral fat reduction
• cognition and neuroprotection
• bone and metabolic health

In our consultations, we discuss BOTH the potential risks and the unique physiologic benefits that oral estradiol may offer compared to transdermal options — because women deserve to understand the full picture and get the best benefits.

We also discuss the fact that randomized controlled trials on oral estradiol have not shown the dramatic risks many women were led to fear decades ago.

This does NOT mean oral estradiol is right for everyone.
And it does NOT mean transdermal estrogen doesn’t have an important role.

Different delivery methods have different risks, benefits, and indications.

But reducing the conversation to:
“patch good, oral bad”
does women a disservice.

Hormone therapy should be individualized — not fear based.

Because hormones change body composition long before most people realize it.

As estrogen, progesterone, testosterone, growth hormone, and insulin sensitivity shift with age, people start losing lean muscle and bone while gaining fat — even if the scale barely changes.

That’s why focusing only on weight can be misleading.

The real goal?
Protecting muscle. Supporting bone health. Optimizing hormones. Improving metabolic health. Aging with strength — not just a smaller number on the scale

In 1979, testosterone below 576 ng/dL was considered low.

Today, 300 ng/dL is considered “normal.”

The reference ranges changed.
But men’s symptoms didn’t disappear.

Fatigue.

Poor recovery.
Low libido.
Brain fog.

Too many men are told they’re “fine” because they fall somewhere inside a lab range.

But “normal” doesn’t always mean optimal.
