¡El dentista de School Smiles llega a nuestra escuela!

Todos los estudiantes inscritos en el programa serán atendidos por el equipo dental el 9/2/26.

Les recordamos que el equipo dental se instalará en la escuela y atenderá a su hijo/a durante el horario escolar.

School Smiles ofrece exámenes, limpiezas, radiografías, flúor y, si es necesario, tratamiento el mismo día. Este tratamiento puede incluir empastes, selladores o SDF.

Si le interesa completar un formulario para SDF o inscribir a su hijo/a para una cita, siga el enlace al sitio web de School Smiles. Si tiene alguna pregunta, llame al 855-497-6453. School Smiles acepta los siguientes planes de seguro para que todos los que quieran inscribirse puedan hacerlo.

Medicaid: Si tiene Medicaid, está cubierto al 100% y nunca se le factura al paciente. Seguro privado: Aceptamos estos planes como cualquier consultorio privado y coordinaremos con las familias para cobrar el copago/deducible correspondiente. Nuestra oficina se encarga de esto.

Opción de autopago: Ofrecemos una tarifa con descuento de solo $49 que incluye limpieza, radiografías, profilaxis, flúor y selladores. Los padres pueden elegir esta opción y la dejamos disponible, ya que algunas familias prefieren pagar en lugar de la opción de subvención.

Fundación de subvención: Si la familia no tiene seguro dental y marca esta casilla, su visita está cubierta al 100% por nuestro programa y nunca se les facturará ningún servicio. Si no tienen seguro, necesitamos que marquen la casilla de autopago o la de subvención para que sepamos cómo proceder, ya que no podemos elegir por ellos. Se puede atender a un máximo de 10 pacientes subvencionados por visita completa. Los fondos de la subvención se otorgan por orden de llegada y se requerirán formularios de verificación de ingresos.

The School Smiles Dentist is coming to our School!

All students who have signed up for the program will be seen on 2/9/26 by the dental team.

As a reminder, the dental team will set up in the school and see your child during the school day.

School Smiles offers an Exam, Cleanings, X-Rays, Fluoride, as well as same day treatment if needed. This treatment can consist of Fillings, Sealants, or SDF.

If you are interested in filling out a form for SDF or signing up your student to be seen please follow the link to the School Smiles Website, for any questions call 855-497-6453. Registrations may be completed online using the following link: https://schoolsmiles.com/student-signup/

School Smiles accepts the following insurance plans so that anyone and everyone that wants to signup can!

Medicaid: if they have Medicaid it is covered at 100%, the patient is never billed.
Private Insurance: we accept these plans just like any private practice and will coordinate with families to collect co-pay/deductible that apply. Our office handles this.

Self-Pay option: we include the discounted rate of only $49 and includes their cleaning, x-rays, prophy, fluoride and sealants. It is the parent’s choice to choose this option and we leave it available because we do have some families that prefer to pay vs having the grant option.

Grant Foundation: If the family does not have dental insurance and they check this box then their visit is covered at 100% by our program and they are never billed for any services. If they don’t have insurance, we do need them to check either the self-pay box or the grant box so that we know how to proceed as we can’t choose for them. A maximum of 10 grant patients can be seen per full visit. Grant funds are also on a first come, first served basis and income verification forms will be required.
