Back Nine Nurses Exclusively Serving Cape Cod Welcome to Back Nine Nurses!

As experienced RNs, Certified Dementia Practitioners, and Certified Geriatric Care Managers, we are here to help families and seniors face the challenges associated with aging. Co-Founders Deborah and Kristen are experienced Assisted Living and Memory Care Registered Nurses as well as Certified Dementia Practitioners. Our business developed in response to a rapidly growing need for trusted, personalized and compassionate navigation for families interested in Assisted Living Communities in the state of Massachusetts. As nurses, we have acquired a wealth of knowledge regarding assisted living resident assessments, service plan development, Massachusetts assisted living regulations and community-specific policies which enable us to optimally advocate for your family. Our Concierge Services include:

• A free 20-30 minute consultation to learn about your unique
situation
• A complete assisted living nursing health assessment
• Identification of the Assisted Living Communities that most
closely align with with your identified needs based on the
assessment, goals, desired geographic location and budget
• Arranging tours, we'll attend together! We customize questions
focused on your specific needs

• Geriatric Care Management Services exclusively for Assisted Living Residents
living on the South Shore of Massachusetts and Cape Cod
• Post-visit review of assisted living community information
• Review of the Initial Service Plan generated by the chosen community
