Flu and other Immunizations are available to students, staff, community members and families. Warren County Health Service Nurses will be here and ready just after 7am Friday October 16th. Students, Staff, Family members and the Community are welcome. The event will be held in the Commons area at Martensdale St Marys School. Please enter through the Central office doors and check in. If you are interested please scan the QR code on the flyer and register with your information.

If more than one type of Immunizations is wanted, please fill out two forms, one for each type of Immunization.
