Free Physicals-Updated
The Ochsner Sports Medicine Clinic will be providing our annual pre-participation physicals on May 16, 2015 free of cost All athletes will need to provide the LHSAA Medical History Evaluation form completely filled out and signed by the athlete’s parent(s) to the clinic on the morning of the examinations. These forms can be found in one of three ways:
Sport specific coach.
Front office.
FHS web page – Athletics.
If the LHSAA Medical History Evaluation form is not filled out prior to the exam, Coach Zimmer will have forms at the sight, but athletes will need a parent with them to fill out and sign the form. Athletes without properly completed forms will not be admitted by Ochsner due to safety/liability concerns. Each team will have coaches present during our scheduled time to assist the athletes. The clinic doors will be locked at 11:45 am to allow the doctors to complete the remaining exams by noon.
