Investigators have released the clearest picture yet of UPS Flight 2976’s final seconds, confirming the MD-11’s left engine and pylon tore away during takeoff rather than failing on the runway. New surveillance images show the No. 1 engine breaking free as the jet rotated from Louisville’s runway 17R, then vaulting up and over the fuselage with flames trailing behind. A second fire erupted at the torn mount on the wing as the aircraft struggled to climb.

The freighter briefly lifted off but never gained meaningful altitude. It cleared the blast fence, then its left gear struck a UPS warehouse roof before the aircraft descended into a storage yard and nearby buildings. Fourteen people were killed, including three crew members, making it the deadliest accident in UPS history.

Early lab work points to fatigue cracking in the pylon’s aft mount lugs, drawing sobering comparisons to the 1979 DC-10 pylon failure in Chicago. The investigation remains active as MD-11 and DC-10 fleets undergo emergency inspections.
