Passport or Consular Card or ID
Immunization Card
High School or GED Diploma (if available)
Transcripts and/or school records
Utility bills, receipts with your name, contracts
Bank statements
Medical records, appts, receipts, bills
Religious documents: baptism, confirmation, 1st communion, marriage, e.g.
Mailing correspondence address to you with postal stamp and dates (if possible)
Any other document with your name and date
